WAIVER AND RELEASE OF LIABILITY

I, _(Name Attached above)_, understand and acknowledge that I am purchasing tire services from Mobile Tire Service LLC, with no warranty, no guarantee, and in AS IS CONDITION. I understand and acknowledge that the tire(s) service(s) I am purchasing will not include wheels to be balanced or tires to be inspected for leaks. I understand and acknowledge that Mobile Tire Service LLC has not inspected the tires and wheels in any way and that no employee, officer, or representative of Mobile Tire Service LLC has made any representations or statements about the condition or quality of the tires. I voluntarily and knowingly release Mobile Tire Service LLC from any and all liability and damages of any kind and any nature in the event of any accident or injury, including death, and including property damage, resulting from or caused in any way by the tire(s) service(s) purchased. I further agree to defend and indemnify Mobile Tire Service LLC  in any action of any kind and any nature brought by any third person against Mobile Tire Service LLC alleging any injury or damages, including death, allegedly caused in whole or in part by any failure or other defect related to the tire(s) service(s)  purchased.
